Initial Comments:
Based on the findings of an offsite follow-up survey completed on September 28, 2022, Lean On Me Home Care, llc, was found tohave corrected the deficiency cited under PA Code, Title 28, Health and Safety, Part IV, Health Facilities, Subpart H, Chapter 611, Home Care Agencies and Home Care Registries. The deficiency was cited as a result of an onsite State Re-licensure survey conducted on April 26, 2022.
